I think it’s fake. If you look at the floor you can see a towel in the reflection from another shot. And his hand doesn’t look right.

I gotta agree that the mid section around his waist and hand is totally photo-shopped and not all that well…the skin tones don’t quite match and that hand is off…Madonna looks off too, but then I did not see the original photos from that set either. Plus, Madonna is in control at all times, I don’t doubt that and this photo wouldn’t exist without her say so I am certain. (But, what do I know? Its just my opinion)
